[__]page_table_check_pte_clear()
This reverts commit aa232204c468 ("mm/page_table_check: remove unused
parameter in [__]page_table_check_pte_clear").
Reinstate previously unused parameters for the purpose of supporting
powerpc platforms, as many do not encode user/kernel ownership of the page
in the pte, but instead in the address of the access.

To provide support for powerpc platforms, provide an addr parameter to the
__page_table_check_ptes_set() and page_table_check_ptes_set() routines.
This parameter is needed on some powerpc platforms which do not encode
whether a mapping is for user or kernel in the pte. On such platforms,
this can be inferred from the addr parameter.

Convert page_table_check_p[mu]d_set(...) to
page_table_check_p[mu]ds_set(..., nr) to allow checking a contiguous set
of pmds/puds in single batch. We retain page_table_check_p[mu]d_set(...)
as macros that call new batch functions with nr=1 for compatibility.
arm64 is about to reorganise its pte/pmd/pud helpers to reuse more code
and to allow the implementation for huge_pte to more efficiently set
ptes/pmds/puds in batches. We need these batch-helpers to make the
refactoring possible.

The repro involved having a huge page that is split due to uprobe event
temporarily replacing one of the pages in the huge page. Later the huge
page was combined again, but the counters were off, as the PTE level was
not properly updated.
Make sure that when PMD is cleared and prior to freeing the level the
PTEs are updated.

Check user page table entries at the time they are added and removed.
Allows to synchronously catch memory corruption issues related to double
mapping.
When a pte for an anonymous page is added into page table, we verify
that this pte does not already point to a file backed page, and vice
versa if this is a file backed page that is being added we verify that
this page does not have an anonymous mapping
We also enforce that read-only sharing for anonymous pages is allowed
(i.e. cow after fork). All other sharing must be for file pages.
Page table check allows to protect and debug cases where "struct page"
metadata became corrupted for some reason. For example, when refcnt or
mapcount become invalid.
